As the federal government and states reform the services they cover and how they pay for care, many Medicaid programs are acknowledging and embracing social drivers of health. Many states are leveraging Medicaid waivers from the Centers for Medicare & Medicaid Services to pay for services that address health-related social needs like housing, transportation, and food and nutrition services. This transition requires building relationships with new provider types and setting up more infrastructure and technology to break down silos and streamline delivery and coordination of care.
